Tip: Please prepare the relevant documents based on the following information depending on the Qualifying Life Event. You must submit these to HR before your enrollment will be approved.
Marriage Certificate
Birth Certificate or proof of hospital birth for newborns
Adoption Papers for adopted children
Divorce Decree
Death Certificate
Employer Letter: If the event is due to a spouse’s change in employment or benefit
1. Navigate to https://benefits.paylocity.com/webbenefits/selfservice
2. Click Change My Coverage
3. Choose the appropriate Life Event from the options in the dropdown list
4. Choose the date that the Life Event occurred
5. Click “Start”
6. Review the information and complete the steps to update your enrollment for your Qualifying Life Event.
